## What this page said

The page said that more than half of the institute's patients were referred by a primary care physician or specialist. Clinicians could hand a patient the institute's materials and contact details, in which case the patient made the first contact and the institute usually asked the referrer to sign a referral form, or, with the patient's permission, fax the form directly to begin intake.

It listed downloadable fact sheets for patients on its programs, including cardiac wellness, symptom reduction, health and fertility, perimenopause and menopause, cancer, chronic pain, weight management and the mothers' program.

## Key points from the original

- More than half of patients came by referral.
- Two routes: patient-initiated or clinician-faxed form.
- Fact sheets were provided for each program.

This is a historical record, not medical advice.
